Histamine measurements are often useful in evaluating patients with suspected anaphylaxis, mastocystosis, gastrointestinal disease, reactions to radiocontrast media, and drug induced anaphlactoid reactions. Additional information may also be obtained by measuring the concentration of tryptase, a mast cell enzyme. Unlike histamine, tryptase is not found in basophils.

Procedure

Enzyme Immunoassay (monoclonal-based competitive binding EIA) after chemical derivatization by acylation (See McBride reference). This test has not been cleared or approved for diagnostic use by the U.S. Food and Drug Administration.

  • Sample preparation is critical: Collect 3 mL of blood in an EDTA tube (lavender top).
  • Cool immediately on ice.
  • Centrifuge at 900 x g for 10 minutes at 4° C. The centrifugation should be performed within 20 minutes of collection
  • Carefully remove 1 mL of plasma from the upper part of the tube and transfer to a plastic vial and freeze.
  • Ship frozen overnight on dry ice. Not stable ambient or refrigerated.
