Placing a Manual Order

  1. In order to begin your manual ordering process, you will need to request a Test Requisition Form from Client Services; please call (800) 305-5198.

  2. Complete the Test Requisition Form and enclose with the specimen (multiple tests may be ordered on one specimen). 
    Test requisitions are updated frequently; please contact Client Services for the most up-to-date test requisition.

  3. Refer to the TEST MENU for collection and shipping requirements. Ensure the package is in accordance with applicable government and IATA regulations for infectious substances.

  4. Recommended shipping method is FedEx®. Use FedEx Clinical Paks or appropriate packing when shipping.
